If you are eligible, we recommend that you make use of your testing accommodations through the testing services provided by the Department for Disability Access and Advising (D2A2)e.g., extended time, distraction-reduced room, reader, transcriber, etc.

For general exams and quizzes, a completed Test Request form (see our Forms web page) must be submitted to D2A2 at least 72 hours (three school days) prior to your requested testing date and time. (See the Accommodated Exam Request Schedule deadline chart on our Policies web page.)

For midterm and final exams, completed Test Request forms must be submitted to D2A2 at least 120 hours (five school days) prior to your requested testing date and time. (See the Accommodated Exam Request Schedule deadline chart on our Policies web page.)

You must submit a completed Test Request form directly to the D2A2 office in order to schedule each exam/quiz. You may deliver a completed hard-copy version of the form in person to the D2A2 office, or you may submit a completed digital (PDF) version of the form via email to the D2A2 office at dss-test@iup.edu.

Before submitting a completed form to the D2A2 office, you must communicate with your professor about the details of the accommodated test/quiz arrangement. Do not assume the professor knows or will be notified. After you have discussed the details of the accommodated testing arrangement with your professor, they must complete the back side (second page) of the Test Request form. This indicates to the D2A2 office that the professor is aware of the details of the arrangement. Only fully completed Test Request forms (both sides), completed by both the student and professor, will be accepted by D2A2.
D2A2 Test Proctoring Procedures

Plan Ahead: See the Deadlines chart (Accommodated Exam Request Schedule) on our Policies web page. Be sure that you leave enough time to get your professor to complete the back side (second page) of the Test Request form.

You must submit a completed Test Request form for each individual test or quiz that you are scheduling to take at the D 2A2 office.

Obtain Test Request forms from the D2A2 office (246 Stouffer, M–F, 8:00–4:30), or download one from the Forms section of the D2A2 website.

You (the student) must complete the front side (first page) of the Test Request form.

When picking a test day and start time, use your class day and time if you have a free period before or after;

Otherwise, on the same day as the class's scheduled test, select a block of time that will allow you to fully make use of your extended time accommodation, which is usually 150 percent/ time-and-a-half (e.g., allow 75 minutes for a test normally scheduled for 50 minutes, two hours for an hour-and-15-minute test).

If school-related scheduling obstacles exist that would prevent you from scheduling your test with extended time for the same day as the class's scheduled test, please communicate with D2A2 and your professor to arrange for an acceptable alternate day or time.

After completing the front side (first page) of the Test Request form, have your professor complete the back side (second page) of the form. (If this is not done, the professor will not know when they must have the test ready for our office to receive, and our office will not have received any special instructions from the professor.)

Make sure the date of pick-up for the test, in the Professor section of the Test Request form (second page), coincides with the time you are requesting to take the test. For instance, if you are taking the test in the morning, the test will need to be obtained by our office the afternoon prior to testing. Or, if you are taking the test in the afternoon, the test will need to be obtained by our office the morning of the same day.

Be sure that your professor notes in the Special Instructions section of the Test Request form if you are allowed to use anything other than a pen/pencil (e.g., such as a calculator, notes, or a book). If this information is not provided on the form by the professor, you will not be allowed to use anything else but a pen/pencil during the testing at our office.

Be sure to submit the completed Test Request form to D2A2 no later than 72 school hours/ three school days (or 120 school hours/ five school days for a midterm or final exam) prior to the day/time that you're scheduling to take the test/quiz at D2A2. Be sure to arrive on time to D2A2 on the day of your scheduled test. If you are more than 15 minutes late for your scheduled test, it will be canceled and you will need to reschedule with both D2A2 and your professor.
