Employer Tuition Reimbursement FormComplete this form and submit it along with any remaining balance due to the Office of Student Billing. A $40 employer reimbursement plan fee will be added to your account. Please download and submit a new form each semester.

Title IV AuthorizationBy completing this form, you are giving permission to the university to pay any outstanding balance on your student account (including non institutional charges) for the same aid year. This will not cover charges from prior aid years.
