Required for the student on Academic Probation to complete at the beginning of each fall and spring semester.
Application for Canceled Semester
Required if the student is being reenrolled to IUP after at least one calendar year and seeking to cancel the effects of one semester's GPA in the cumulative GPA calculation.
Application for Fresh Start
Required if the student is being reenrolled to IUP after at least two calendar years and seeking to cancel the mathematical effects and credits earned from any D or F grades on the transcript.
Application For Dual Baccalaureate Degree
Required for the student seeking a dual degree. The form needs to be completed for the college of the secondary baccalaureate degree. If the secondary baccalaureate degree is in the COEC, please complete this form.
Application for Double Major
Required if the student is declaring a second major, but does not intend to pursue a dual baccalaureate degree.
Application for Catalog Year Change
Required if the student would like to change the catalog year (major requirements) to a year other than the one assigned when first admitted to IUP.
Application for Name Change - Correction
Required if the student wants to change their name (e.g., marriage) or correct their name on their official banner profile.
Application for IUP Graduate Early Admittance
Required for the student who has been admitted to a graduate level program and wants to take graduate courses, while still an undergraduate student.
Required for the student who wants to take a graduate course(s) to count toward the undergraduate major, but has not been accepted early into a graduate level program.
Application for Excess Academic Load
Required if the student wants to take more than 17.9 credits in the fall, spring, or summer semesters or 6 credits in winter session. Advisor signature may be needed.
Required for the student who seeks to substitute a course not listed as a Liberal Studies option for the required course.
Application for Total University Withdrawal and Checklist
Required if the student wants to withdraw from all classes within a given semester before the deadline or after the deadline has passed.
Application for Undergraduate Graduation
Required if the student is not able to apply for graduation online through MyIUP.
Application for Visiting Student Status
Required for the student who wants to take one or more courses at any of the other 13 PASSHE institutions.
